The first thing you need to do is become aware of excess tension in your body when you play. To do that, stop yourself in the middle of playing & do a mental scan of your body looking for tense areas or flexed muscles & intentionally relax those spots. It's that simple, but it takes some practice to get it down & get rid of tension that's holding you back.
